At Dr Mannan IVF Centre in New Delhi, one of the most common and challenging conditions I treat is endometriosis.
With over 10 years of experience in fertility care, I have seen firsthand how difficult this condition can be for women.
For too many, the journey to a diagnosis is long and frustrating. They suffer for years with debilitating period pain, chronic pelvic discomfort, and fertility struggles, often being told their symptoms are just “bad cramps” or “part of being a woman.”
I want to be very clear: severe pain is not normal. Endometriosis is a real, complex, and often painful disease, but it is treatable.
My mission is to validate your experience, provide an accurate diagnosis, and create a personalised treatment plan that aligns with your goals—whether that is living pain-free, restoring your quality of life, or achieving your dream of motherhood.
To understand endometriosis, we first need to look at the endometrium—the tissue that lines the inside of your uterus.
Every month, this lining thickens and then sheds, causing your period.
In endometriosis, tissue similar to this lining grows outside the uterus. These rogue implants can be found on the ovaries, fallopian tubes, the outer surface of the uterus, and the lining of the pelvic cavity.
The problem is that this misplaced tissue still behaves like the uterine lining: it thickens and bleeds with your monthly cycle.
But unlike your period, this blood has no way to exit the body.
This internal bleeding leads to inflammation, intense pain, and the formation of scar tissue (adhesions) that can bind organs together.
When endometriosis forms on the ovaries, it can create cysts filled with old, dark blood, known as “chocolate cysts” or endometriomas.
The symptoms of endometriosis can vary widely, but the most common signs include:
The severity of your pain does not always correlate with the extent of the disease.
Some women with severe disease have little pain, while others with mild disease experience debilitating symptoms.
Diagnosing endometriosis requires a high index of suspicion and the right tools.
Treatment is tailored to your symptoms, the severity of the disease, and your desire for children.
1. Medical Management
For managing pain, hormonal medications can be effective.
2. Laparoscopic Surgical Treatment
For women with significant pain, large cysts, or infertility, surgery is often the best option.
The goal of surgery is to remove the disease, restore normal anatomy, and reduce pain.
For women trying to conceive, surgery can significantly improve the chances of a natural pregnancy.

No. Endometriosis is a benign (non-cancerous) condition. However, it can be very invasive and cause significant damage to pelvic organs.
Laparoscopic surgery can remove the existing disease, but as long as you are menstruating, there is a risk it can recur. The only definitive cure is a hysterectomy with removal of the ovaries, which is a last resort for women who have completed their families and have severe, unresponsive disease.
For many women, yes. Surgery can improve fertility by removing scar tissue, unblocking tubes, and reducing inflammation. We often recommend trying to conceive naturally for 6-12 months after surgery, as this is the window of highest fertility.
Recovery is quite fast. You can typically go home the same day or the next day. Most women are back to their normal routine within 1-2 weeks.
Absolutely. This is a very common scenario. Superficial endometriosis implants are not visible on ultrasound. If your symptoms are classic for the disease, a diagnostic laparoscopy is the next logical step to get a definitive answer.
