As an IVF and fertility specialist with over 10 years of experience, I, Dr. Mannan Gupta, often consult with young women who have just received a devastating diagnosis: Premature Ovarian Failure (POF), also known as Primary Ovarian Insufficiency (POI).
At my clinic in Delhi, I understand that learning your ovaries have stopped functioning before the age of 40 can feel like a profound loss—of your fertility, your sense of self, and your dreams of motherhood.
The first thing I want you to hear is that while this diagnosis is life-altering, it is not the end of your story. It is a complex condition that requires a dual approach: managing the health implications and, most importantly, navigating your path to parenthood.
My commitment is to guide you through this journey with medical expertise, deep compassion, and a clear-eyed view of the options that modern fertility science can offer.
Premature Ovarian Failure, or Primary Ovarian Insufficiency (POI), is a condition where a woman’s ovaries stop functioning normally before the age of 40.
This means the ovaries are no longer releasing eggs regularly and are not producing enough of the hormone oestrogen.
It is crucial to understand that POF is not the same as early menopause. In menopause, periods stop completely.
In POF, about 5-10% of women may still have intermittent ovarian function and can occasionally get a period or even conceive. However, for most, it marks the end of their natural reproductive window.
In over 90% of cases, the exact cause of POF is unknown (idiopathic). However, for a small percentage, we can identify a specific reason:

The symptoms of POF are essentially the symptoms of low oestrogen, similar to what women experience in menopause. They include:
If POF is suspected, the diagnosis is confirmed through simple blood tests and a detailed history.
Treating POF is about managing your immediate health and planning your future family.
1. Hormone Replacement Therapy (HRT)
This is not about fertility; it is about your long-term health. The loss of oestrogen at a young age significantly increases the risk of osteoporosis (thinning bones) and heart disease.
HRT replaces the hormones your body is no longer making, protecting you from these risks. It is a vital part of managing your overall well-being and is typically continued until the natural age of menopause (around 50-51).
2. Building Your Family: The Path to Parenthood
For most women with POF, conceiving with their own eggs is highly unlikely. The most effective and successful path to parenthood is through Donor Egg IVF.
The cost of managing POF involves two components: the diagnostic and health management aspect, and the fertility treatment aspect.
Service / Treatment | Approximate Cost in Delhi |
Hormonal Blood Tests (FSH, Oestrogen, AMH) | ₹3,000 – ₹6,000 |
Genetic Testing (Karyotype) | ₹4,000 – ₹7,000 |
Hormone Replacement Therapy (HRT) (Monthly) | ₹500 – ₹1,500 |
Donor Egg IVF Cycle | ₹2,00,000 – ₹3,50,000 |
Understanding the Cost Variation:
The cost of a donor egg IVF cycle can vary significantly. This depends on whether you use eggs from an anonymous donor from an egg bank or a known donor, and the specifics of the clinic’s programme.
We provide a fully transparent cost breakdown that covers donor screening, compensation, the IVF procedure, and your medication.

Unfortunately, there is currently no proven medical treatment that can reverse POF or restore normal ovarian function. The focus of treatment is on managing the health consequences and using donor eggs for fertility.
For the vast majority of women with POF, this is not possible as there are no viable eggs left. Donor egg IVF allows your partner to be the biological father, and you have the invaluable experience of carrying the pregnancy and giving birth.
For young women with POF, HRT is not only safe but essential. You are simply replacing the hormones that your body should be producing at your age. The benefits for your bone and heart health far outweigh any risks.
Our egg donors are young, healthy women who undergo extensive medical, genetic, and psychological screening. We match donors to you based on physical characteristics to ensure the best possible resemblance. The process is completely anonymous and ethical.
DOR is a decline in egg quantity, often age-related, but the woman still has regular cycles and produces oestrogen. POF is a more complete cessation of ovarian function, with irregular or absent cycles and low oestrogen levels. DOR can sometimes lead to POF.
